V55/5 Example Images
The purpose of this page is to aid you in completing a V55/5 form.
Point to note: The VIN number must correspond to what is on the HMRC database and the dating letter.
I have Lambrettas with various VIN numbers. A VIN number may be made up of just several numbers, for example 12345, or numbers / engine size and lambretta type, for example 125LD*12345. It is important that The VIN number corresponds to what is on the HMRC database and the dating letter. If they do not patch, your application will be rejected.
If your V55/5 form is rejected due to incorrect NOVA mismatch issues, the DVLA will inform you to call the HMRC Personal Transport unit 0300 3227071 to update the information. (Reclaiming back an old number plate)
Note – Not seen in the image below. If reclaiming an old number plate with the use of a RF60 or VE60 old style log book, you should enter this information in Field 1

Note: The first registration date and first UK registration date is the date that the DVLA assigned the number plate. Page one / item 3 – Special notes states:
1: Non transferable registration mark
2: Was registered and or used: Declared manufactured 1954. This statements aids with the historic vehicle not having to have an MOT
